3209 - Hepatitis B Vaccinations

All employees who are employed in the following listed positions shall be offered a vaccination against the Hepatitis B virus in whatever manner is determined by the City to be most appropriate. If an employee elects not to have the appropriate vaccinations against the Hepatitis B virus, they must sign a "Hepatitis B Vaccination Waiver Form" releasing the City from any liability with regard to their contracting the Hepatitis B virus and any and all costs associated with their contraction of that disease.

The employees who will be affected by this policy are those in the following positions: Fire Chief, Assistant Fire Chief, Fire Marshall, Fire Shift Supervisors, Fire Station Officers, Fire Inspector, all Fire Fighters (including paid-on-call), all Sworn Police Officers, Crime Scene Analyst Supervisor, Crime Scene Investigator, Evidence Technician, and Code Enforcement Officer I/II. The authorization form and directions for receiving this vaccination are available in Police and Fire Department policies.

The City recommends that the above employees also receive the Hepatitis A vaccination, however, this is not mandatory. Employees desiring to be vaccinated for Hepatitis A may do so at their own expense, and make their own arrangements with the Davis County Health Department or their personal physician. The City may arrange to provide this vaccination at the work place from time to time.
