COVID-19 UPDATE - Passport Services Temporarily Suspended

In coordination with Governor Herbert’s recommendation to limit public gatherings, and with an abundance of care for the health and well-being of our residents, guests, and staff, Layton city will suspend passport applications at the city. As of Wednesday, March 25, 2020 this service will be suspended until further notice.

Layton City is a Passport Acceptance facility designated by the U.S. Department of State's Bureau of Consular Affairs.

Passport Hours

TEMPORARILY SUSPENDEDProcessing hours are from 8 a.m. to 4 p.m. Monday through Friday (closed major holidays). No appointment necessary.  Please arrive prior to 4 p.m. for same-day application processing. Applicants are encouraged to complete all application forms prior to arriving at City Hall.


  • All applicants must appear in person at time of application.
  • Applications for children 16 and 17 years of age require the presence and signature of one parent.

Applications for children under 16 require the presence and signature of both parents. If both parents are not available to sign, please see additional requirements at (reference step 7. Show Parental Consent) .


These are the items and documents you should bring with you to submit your passport application.

  • Completed application DS-11
  • A color passport photo - must be taken within the last six months and meet government requirements.
    • Photo service available at Layton City for $10 each plus tax.
  • Proof of Identity, such as:
    • Government, military, or state identification card
    • Previous passport
    • Valid driver license
    • Parent's identification for a child's passport (under age 16)
  • Proof of Citizenship:
    • Original or Certified Copy of Birth Certificate (hospital issued birth certificates are not accepted)
    • Original Certificate of Naturalization
    • Original Certificate of Citizenship
    • Previous U.S. Passport book or card
    • If a child under 16 has a previous passport, additional Evidence of Citizenship is still required.
  • Social Security Number
  • A check (personal, certified, cashier's) or money orders made payable to 'U.S. Department of State' for the application fee (one for each application).
  • Cash, check, credit or debit card to pay for your Acceptance Fees to Layton City.


You can expect to pay the following fees when applying for your Passport. Expedited service will not be available and routine service may be delayed.

A 2% surcharge will be assessed on all credit card transactions over $75. The surcharge is not greater than our cost of acceptance.
